Instructions for patients requiring Travel Vaccinations

If you require travel vaccinations at the surgery you will need to complete a travel form listing the countries and the areas you will be visiting prior to booking an appointment.

It is important to make an initial appointment as soon as possible, approximately 6 weeks before you travel, as a further appointment will be required with the Practice Nurse for the vaccinations to be administered. These vaccines are not a stock vaccine and will also have to be ordered before your 2nd appointment. Please also be aware that certain vaccines take 2 weeks to take effect and that some travel vaccines are ordered on a private prescription and incur a charge above the normal prescription charge, as they are not included in the services provided by the NHS.

  • The Practice Nurse will contact you within two weeks to discuss your vaccination plan and advise you of the cost of your treatment - this will need to be paid BEFORE your appointment. Please remember to include a contact telephone number on your travel form.
  • If you do not have access to a computer or email, please contact the surgery to collect a travel form from reception.
  • These appointments will be held at The Culverhay Surgery.
